You will need to be passionate about supporting our community in Blackpool, be committed to your role as a Social Worker and keen to grow and develop in line with Social Work England protocols. This is a hybrid role and you will be required to attend the office 3 days per week - staff parking is available at reduced rates.

You will construct value-for-money care packages, and commission and arrange appropriate services.

You will be responsible for the completion of Care Act assessments, capacity assessments and Court work - section 21a challenges. Requirements of this role: - Must be a Qualified Social Worker, registered with Social Work England.

You must understand and actively participate in the department’s safeguarding adults’ procedures, including identification, investigation, and taking the role of Safeguarding Lead at H2.

You will work closely with other teams, including undertaking joint assessments with providers, carers, and external agencies to deliver outcome-focused services to individuals and their families.

You will need to have experience of working in the field of learning disabilities, mental health and autism, and have experience of leading and managing teams. You will coordinate the allocation of safeguarding referrals and monitor their progress.

You will also request funding for packages of care via the resource allocation group through weekly presentations and negotiations.

You will contribute to team duty rotas to provide prompt and emergency support as required.

You will support service users in various activities, including employment, volunteering, community engagement, and daily living tasks at home. We are looking for someone with a great sense of humor who enjoys being part of an energetic team.

You will work collaboratively within a multi-disciplinary team, including Adult Social Care and NHS Health staff. Managing safeguarding cases, assessing risk and coordinating care management effectively are key responsibilities.

You will engage with service users, families and partner agencies to promote person-centred support.

You will be expected to complete Care Act assessments and reviews, Social Circumstances Reports and Mental Capacity Assessments.
