Candidates should have QCF Level 2 in Health and Social Care and at least 18 months of care experience. Excellent communication skills and the ability to respond flexibly to service needs are essential.

You will provide continuous 1:1 nursing care, medication management, monitoring of chronic conditions and coordination with local doctors and clinics.
