You will work closely with young people to understand their individual needs, contribute to holistic assessments, and develop, review, and implement effective Pathway Plans as they prepare to move on from care. The role also involves providing tailored advice, guidance, and practical support to meet each young person's unique circumstances.

You will have a set weekly rota made up of the below shifts. (Usually 2 Early shifts and 1 x Mid , 1x Late)
