You will be able to demonstrate an understanding of the needs and risks our customers experience and a commitment to helping customers develop and identify their recovery goals.

You will work with people to positively develop strategies to enable them to better manage their mental health and sustain their housing.

You will lead on high-risk cases, support less experienced Social Workers, and act as a Practice Teacher for students on placement. Key Responsibilities

You will work collaboratively with other professionals to ensure the safety and well-being of service users, addressing their needs through a person-centred approach. Additionally, you will be expected to handle complex cases, provide crisis intervention, and ensure compliance with relevant legislation and policies.

You will be employed directly by the client, however, CCMS oversee all aspects of the clients care package on her behalf. CCMS offer a bespoke Case Management Service for people of all ages, with a particular emphasis on those recovering from acquired brain injuries.
