You will translate clinical information from patient records and must ensure accuracy and adherence to deadlines. Competitive salary ranges from £38,682 to £46,580 annually, based on experience and qualifications.

You will conduct adult-only functional assessments across a wide range of physical, neurological and mental health conditions.

You will complete a fully paid 7-week training programme, designed to provide the knowledge, confidence and competence required for the Functional Assessor role.

You will also deliver personalised, evidence informed medical care to all patients in contact with OoHs and provide advice, treatment and onward referral as appropriate.

You will adhere to local prescribing guidelines and keep good quality up to date medical records including ensuring appropriate documentation and coding is used to provide consistent audit and monitoring of the service.

You will be an integral part of the multidisciplinary team, which comprises of Nurses, Doctors, Occupational Therapists, Psychologists and a range of non-registered health care roles.

You will work within a defined scope of practice and to agreed clinical standards applied within the local setting.
