You will collaborate with children, parents, schools, health services and partner agencies to ensure effective, child‑centred planning.

You will look after children and young people with disabilities aged 0 to 18

You will need to have knowledge of, and a working understanding, of the Children Act 1989, the Children and Families Act 2014 and the Care Act 2014

You will work with young people to prepare them for the transition to adulthood and support them through the process

You will be expected to be take a flexible approach and potentially could be expected to commute outside of your specified borough. This commute would be covered within our Mileage rate within our policy. Due to the requirements of this role we are happy to also consider applications from outside of the posted location.
