You will be joining the West D2A Beds Discharge Team, supporting timely discharges and helping prevent delays for Staffordshire patients placed in Discharge to Assess (D2A) beds across private care homes commissioned by MPFT. You'll work closely with a Multi-Disciplinary Team, including:
