You will be working within a specialist multi-disciplinary team supporting people with a severe and enduring mental health condition. To have a responsibility for a caseload and fulfilling statutory duties such as undertaking assessments, developing Care & Support Plans, and completing reviews, including Section 117 reviews.

You will work with people in crisis, where urgent support arrangements are needed, and make decisions regarding mental capacity and/or the Mental Health Act. Additionally, you will support individuals to receive care and treatment in the community rather than inpatient settings, arranging effective discharge pathways and facilitating community connections, housing, and discharge arrangements.

You will collaborate with a team of experienced practitioners, utilising evidence-based interventions to promote social inclusion and support mental health recovery. The team is a trust wide service covering all five boroughs of Pennine Care and it is essential that you drive to enable you to travel across the Trust footprint.

You will be accountable for complex decision making in relation to caseloads ensuring professional judgement, service user involvement, needs led assessment, critical reflection, and analysis to inform this.
