You will develop and deliver customer-focused programs, ensuring compliance with policies while promoting community participation. The ideal candidate has experience in service delivery, excellent interpersonal skills, and a strong commitment to equality and diversity. This role requires working various shifts as necessary.

You will manage a team of Community Care Workers, ensure compliance with health care standards, and handle referrals and staff coordination. Candidates should have QCF Level 2 in Health and Social Care and at least 18 months of care experience. Excellent communication skills and the ability to respond flexibly to service needs are essential.

You will be confident in decision making and have the knowledge and understanding to step in when needed to help resolve issues and drive performance.

You will share best practice and key learnings to really make the difference with what we do.
