You will provide high-quality assistance to service users in their daily lives, encouraging personal development and community engagement. You will monitor adherence to policy and procedure and ensure that our values are always upheld.

You will provide direct person-centered care to our valued and diverse group of service users.

You will be expected to work as part of the on-call team on a rota basis.

You will contribute to the delivery of care in accordance with the Health & Social Care Act 2008.

You will play a crucial role in the initial stages of child safeguarding by completing high-quality assessments that inform timely, proportionate, and effective decision-making.

You will be required to be office-based for the first four weeks to support induction and embed the MK practice model and values.

You will monitor adherence to policy and procedure and ensure that our values are always upheld.

You will provide direct person centered care to our valued and diverse group of service users.

You will be expected to work as part of the on-call team on a rota basis.

You will contribute to the delivery of care in accordance with the Health & Social Care Act 2008.

You will play an active role in the discharge of our service users from hospital and will commission health and social care services as required. Partnership working is a key component of this post and you will need to be innovative in ensuring that there is a range of appropriate services locally to support individual recovery journeys.
