You will work with people in crisis, where urgent support arrangements are needed, and make decisions regarding mental capacity and/or the Mental Health Act. Additionally, you will support individuals to receive care and treatment in the community rather than inpatient settings, arranging effective discharge pathways and facilitating community connections, housing, and discharge arrangements.

You will look after children and young people with disabilities aged 0 to 18

You will need to have knowledge of, and a working understanding, of the Children Act 1989, the Children and Families Act 2014 and the Care Act 2014

You will work with young people to prepare them for the transition to adulthood and support them through the process

You will work with adults aged 18+ on assessment and support planning.

You will also need to conduct mental capacity assessments and handle safeguarding responsibilities.

You will be an effective multi-tasker with excellent organisational skills and the ability to work flexibly to meet the needs of the service.

You will be accountable for complex decision making in relation to caseloads ensuring professional judgement, service user involvement, needs led assessment, critical reflection, and analysis to inform this.

You will identify and assess levels of risk and need (within statutory frameworks) in often complex situations.

You will work alongside the Community Mental Health Teams to provide social care to individuals with moderate to severe mental health problems.

You should have the ability to work from the office Monday to Friday and visit service users in their homes or supported accommodation .
