You will undertake assessment and care management and other Social Work responsibilities including liaison with health, private and voluntary agencies.

You will also monitor, review and assess individual care plans and respond to changing needs.

You must hold a Diploma in Social Work or equivalent related Social Work qualification.

You will be responsible for preserving the good condition of the infrastructure and ensure that the Facilities are safe and well-functioning.

You will be required to work on own judgement and decisions, communicate internally and externally as well as managing contractors daily activities.
