You will develop and implement personalised care plans, provide crisis intervention support, and manage a caseload effectively.

You will work closely with reablement services to maximise independence following hospital discharge or within a community setting, ensuring compliance with all relevant legislation, policies, and procedures.

You will handle cases transferred from Locality Safeguarding, including those with no recourse to public funds, designated CIN LA, or SGO support plans.

You will also manage cases involving babies from the pre-birth pathway, Section 7 and 37 private law proceedings, and pregnant teenagers or young parents.

You will provide consultative guidance to the team, ensuring robust decision-making processes aligned with best practice guidelines. Conduct assessments to determine the needs of adults requiring support. Collaborate with other professionals to develop personalized care plans.
