You will undertake specific patient care within the District Nursing Team as delegated by the Registered Nurse.

You will visit several patients daily in their own homes

You will provide high quality nursing care to a range of patients with varying healthcare needs. Provide a high standard of holistic assessment and care planning.

You will be expected to provide wound management, health promotion, medicines management, end of life care

You will be required to work collaboratively in a safe, caring and respectful way.

You will contribute to enhancing the quality of the practice learning environment by providing support, educational input and development activities for the current and future community nursing workforce inclusive of registered nurses and HCSW.
