You will be part of a dynamic community-based team that ensures that families requiring additional support are identified and helped to access early help at the earliest opportunity.

You will be an integral part a team that is being created to provide families across Cheshire West and Chester with early help at the earliest opportunity.

You will work with young people, families, and carers creatively to deliver a wide range of evidenced-based interventions to meet their support needs.

You will have experience of completing Whole Family Assessments and be adept at creating support plans using SMART goals.

You will take a multi-agency approach and work collaboratively with a wide range of internal and external services.

You will be required to manage a multi-disciplinary team to support vulnerable families to access and engage with Family Support Services through a locality model of working.
