You will provide clinical leadership to a skilled multidisciplinary team, oversee assessments and care planning, and work closely with community CAMHS, Eating Disorder, and Neurodevelopmental teams. There are opportunities for research, teaching, and service development, with strong support for CPD and flexible working.

You will be responsible for assessment, planning and implementation of care and assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs and set realistic goals.

You will need excellent organisational skills along with a wide knowledge base and experience of working in older person's services.

You will provide frontline care and management of patients presenting with undifferentiated minor injuries and ailments, working collaboratively with other healthcare professionals to ensure the delivery of safe, efficient, and patient-centred care. Our Minor Injuries Units are located across the County of Cornwall and the Isles of Scilly and are waiting to welcome you into the team.

You will provide a clinical review of patients and work collaboratively with an established multi-disciplinary team and senior nursing colleagues. Keeping frailty patients closer to home where appropriate is a key NHSE aim and is transforming the management of patients outside an acute hospital setting.

You will lead the creation of a centralised Frailty Learning Hub, bringing together system-wide training offers and resources. The hub will support both registered and non-registered workforce across acute, community, primary care, social care, VCSE, and care home settings.
