implementation specialist clinical jobs in County Durham

You will be based at your local Apollo Home Healthcare office in Spennymoor, but some flexibility is required as you will be travelling to clients homes across the North East region. In this role, you'll: - Take clinical ownership of your service user caseload, creating and managing bespoke care plans.

You will assist with the team in implementing a comprehensive assessment process and a range of time limited therapeutic interventions, working within a multi-disciplinary team.

You must also be able to work on your own initiative, you will need to demonstrate excellent communication, facilitation, organisation and leadership skills. Promote excellence and contribute to programme reviews, audit and share the necessary outcomes and support the implementation of findings.

You will be required to effectively and efficiently deal with stakeholders, manage multiple roles and stick to tight deadlines, all while providing a world class service. This role requires flexibility and the ability to quickly adapt your working style to the different clients & sites at short notice.
