You will also have responsibility for those service users under the MHWT without Care coordinators or key workers. Your responsibility will be to ensure risk assessment are updated, medical review is booked, and physical health screening has been carried out.

You will carry out screening, risk assessment and safety planning, alongside providing ongoing emotional and practical support to victims and survivors.

You will also work with children and young people impacted by domestic abuse, ensuring their needs are identified and met through appropriate emotional and therapeutic support.

You will deliver face‐to‐face services, enabling patients to obtain health information from the appropriate health care professional.

You will be providing safe and effective health assessments and treatment of patients within the establishment.

You will work within a clinical team consisting of GPs, UCPs, Nurse Streamers and HCAs to deliver excellent care services within our UTC.

You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

You will have the opportunity to be part of our mission to make health confidence accessible to all.

You will see 15 to 22 customers daily, with appointments ranging from 10 to 45 minutes depending on the package.

You will perform various health assessments such as PAD, ECG, Lung Function, Body Composition, and fitness tests, and offer advice on further testing if appropriate.
