You will collaborate with a team of experienced practitioners, utilising evidence-based interventions to promote social inclusion and support mental health recovery. The team is a trust wide service covering all five boroughs of Pennine Care and it is essential that you drive to enable you to travel across the Trust footprint.

You will link closely with both inpatient teams and home treatment teams, ensuring a seamless care approach, and will also work jointly with primary care pharmacy teams.

You will cooperate in ensuring the provision of high‐quality, patient‐focused pharmaceutical care.
