You will be part of an expanding team based in Lancing and the role will predominantly be one of telephone triage, liaison and coordination of existing and new patients registered to the ECHO service. The service covers seven days a week, 24 hours a day, 365 days a year.

You will deliver the Individual Placement and Support (IPS) model; providing person centred guidance to clients, whilst building positive relationships with local employers to enable clients to move into suitable employment.

You will apply case management principles to support patient self‐management, promote independent living, and reduce hospital admissions through robust contingency planning.
