You will enable young people to achieve positive outcomes and independence by supporting the development of occupational performance skills, prescribing equipment, minor adaptations, and supporting within our Early Help model in the Transitions hub.

You will work closely with internal and external agencies and departments across all Adult Social Care.

You will be joining an outstanding set‐up within a well‐known Community Rehabilitation Team in Bristol. Please note that due to the community nature of the role, you will be required to be a car driver and owner. The hourly pay rate for this role is £23 - £25 P / H.

You will be responsible for managing a caseload of patients with complex needs following an acute neurological event such as traumatic and acquired brain injury, or other neurological conditions.

You will ideally have clinical experience in the assessment and management of patients experiencing cognitive and physical impairments and subsequent difficulties in function.
