You will conduct complex assessments of individuals’ needs and circumstances.

You will develop and implement comprehensive care plans and deliver targeted interventions to support individuals in managing their needs. Additionally, you will work collaboratively within a team that covers half of the Borough and undertake assessments face-to-face in a hybrid working environment.

You will be working with a diverse group of people, each with their own unique abilities and needs. From assisting with daily activities to providing emotional support, you’ll play a vital role in their life. The successful candidate must be female, and preferably a driver although this is not essential.
