You must have a driving licence and access to a vehicle to carry out your duties. Work a shift pattern covering 7 days a week from 08:00‐21:00. Qualifications and experience Experience in adult community services is desirable.

You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing.
