You will work within the framework of the Care Act 2014 and undertake timely assessments and reviews. This will be a short term position of 8 weeks and Mental Health experience with older adults or an understanding of mental health legislation is preferable Key Responsibilities:

You must understand and actively participate in the department’s safeguarding adults’ procedures, including identification, investigation, and taking the role of Safeguarding Lead at H2.

You will work closely with other teams, including undertaking joint assessments with providers, carers, and external agencies to deliver outcome-focused services to individuals and their families.

You will coordinate the allocation of safeguarding referrals and monitor their progress.

You will also request funding for packages of care via the resource allocation group through weekly presentations and negotiations.

You will contribute to team duty rotas to provide prompt and emergency support as required.

You will work collaboratively within a multi-disciplinary team, including Adult Social Care and NHS Health staff. Managing safeguarding cases, assessing risk and coordinating care management effectively are key responsibilities.
