Additionally, you will support individuals to receive care and treatment in the community rather than inpatient settings, arranging effective discharge pathways and facilitating community connections, housing, and discharge arrangements.

You will conduct comprehensive assessments of clients’ mental health needs, strengths, and challenges.

You will provide individual, group, and family counseling to help clients manage mental health issues.

You will coordinate services and resources for clients, including housing, healthcare, and financial assistance.

You will work with people in crisis, where urgent support arrangements are needed, and make decisions regarding mental capacity and/or the Mental Health Act. Additionally, you will support individuals to receive care and treatment in the community rather than inpatient settings, arranging effective discharge pathways and facilitating community connections, housing, and discharge arrangements.

You will be developing clear support plans following Care Act assessments, completing funding applications, and managing legal requirements such as COPDOL, DoLS, 21a’s, and court reports. Additionally, you will build meaningful relationships with individuals often considered ‘difficult to engage’ and their families.

You will primarily be responsible for handling incoming safeguarding referrals. The ideal candidate will have relevant experience and the ability to work in Rochdale.
