You will be a registered practitioner with experience ofminor illness, such as a GP or a prescribing advanced practitioner.

You will achieve high quality, timely and person‐centred needs assessments, plan and work to improve outcomes, and increase or maintain the independence of vulnerable adults.

You will work within a multidisciplinary team, supporting patients with acute mental health needs and conducting risk assessments. Essential qualifications include being a Registered Mental Health Nurse or Occupational Therapist with a minimum of 3 years' experience in mental health.
