You will work with participants at the PACE Center and in their homes addressing issues ranging from high risk mental health, end of life, medical decision making, caregiver support, and more.

You must have an MSW and be passionate about serving adults 55+. Experience working with older adults preferred.

You will be expected to apply a strengths-based approach using the Three Conversations Model. The ideal candidate will bring a positive attitude and have experience managing complex cases and situations. This role requires a high level of skill in: AssessmentPlanningInterventionEvaluation
