You will work across the North / Perth / South locality of Perth and Kinross, this includes areas such as:

You will have the skills that allow you to communicate effectively with individual clients, internal / external agencies, and the staff team.

You must also have the ability to maintain confidentiality, work co-operatively with other professionals, families and carers.

You will have an ability to undertake robust assessments and care planning which will enable you to update care plans to ensure the outcomes of the individuals are being met, as well as having the flexibility and skills to respond to changing needs of individuals.

You will have good organisational skills and be able to manage a caseload.

You will have access to training and support for continued professional development.

You will be required to provide this check.
