You will construct value-for-money care packages, and commission and arrange appropriate services.

You will be responsible for the completion of Care Act assessments, capacity assessments and Court work - section 21a challenges. Requirements of this role: - Must be a Qualified Social Worker, registered with Social Work England.

You will be working Full time (42 hours per week) across a mixed shift pattern of 3/4 12 hour shifts per week with a focus on supporting our d/Deaf (profound, severe, partial or hard of hearing) service users with mental health needs, inside our new Deaf community step down setting in Blackburn.

You will be part of a co-located team working with people with severe and enduring mental health conditions in the Blackburn with Darwen area.

You will be expected to have a basic understanding of the Mental Health Act 1983, with opportunities to develop your learning through a generous training programme.

You will also safeguard vulnerable adults from abuse, neglect, and exploitation through investigation and collaboration with other agencies. Additionally, you will conduct thorough assessments and develop personalized care plans for adults in need.
