You will be the point of contact for the company in relations to all projects ensuring all processes are followed and ran smoothly. You will have experience working within Mental Health Care and Support Teams, supporting adults with mental health needs. As the post-holder, you will undertake social care assessments and reviews under the Care Act 2014 and hold a caseload of service users requiring complex case management.
