You will develop personalised care and support plans in collaboration with individuals, their families, and other professionals while applying a strengths-based approach to promote independence and well-being.

You should have post-qualified experience in adult social care, particularly in working with individuals with cognitive and physical disabilities.

You must be over 18 years old and have the right to work in the UK. Flexibility in your availability, with the ability to travel to different care settings. A good standard of English and the ability to communicate effectively. A clear enhanced DBS, which you can apply for upon registration.
