You will be able to construct a person centred support plan, implement, review and evaluate its effectiveness for the service users for whom you have responsibility.

You will contribute to the assessment and review process and administer medicines as prescribed. Shift and weekend working is essential.

You will be responsible for providing direct support to individuals in a variety of settings.

You will play a key role ensuring that individuals receive high standards of care and support to help maximise their independence and dignity throughout their lives.
