Under the leadership and management of the Team Manager/Principal Social Worker to provide a statutory social work service to adults with health and social care needs achieving high quality, timely and person-centred needs assessments that will reflect the needs and risks to the individual, analysis, care planning, intervention and review appropriate to the work of the team or service unit....

You will actively promote self-learning to ensure professional development and provide high-quality practice in line with the Professional Capability Framework (PCF).

You will work with other team members and multi-disciplinary team members from other agencies to meet the needs of vulnerable adults.

You will carry out statutory social work functions including assessments, support planning, reviews, and safeguarding work. The ideal candidate will be confident working independently, managing their own caseload, and collaborating within a multidisciplinary environment. Key Responsibilities

You will need to be able to attend three days training if successful which is scheduled for end of January so this role commences beginning of Feb. Experience in 2:1 or 1:1 intensive support packages and or NVQ Level 2 or 3 in Health & Social Care would be desirable

You must possess full right to work in the UK and not require sponsorship to be considered for this post.
