You will complete information gathering on the wards.

You will hold MDT (multidisciplinary team) conversations with professionals to decide the pathway for a safe and timely discharge. Additionally, you will be able to respond to A&E and plan for care from the hospital.

You will also play a key role in promoting independent living, developing staff competencies, and driving innovation within therapy services across the region.
